Metro South Health are offering free advance care planning (ACP) clinics occurring at Community Health Centres during 2024.

The clinics, hosted by the Metro South Health ACP Service team will enable people to learn more about ACP and receive support to complete Queensland ACP documents including the Enduring Power of Attorney, Advance Health Directive and Statement of Choices. 

Christine Duncan, Nurse Unit Manager of the Metro South Health ACP Service team says that ACP supports people of any age or stage of health to plan their health care, in advance, by sharing their personal values, goals and health care preferences. 

“ACP conversations and documents allow health care preferences to be known and shared and are used in situations where a person is unable to communicate their wishes for themselves,” Christine says.  

“This means loved ones won’t have to make difficult health care decisions in a crisis situation on a person’s behalf without knowing what the person would have wanted.” 

People attending the clinics can benefit from the availability of experienced Clinical Nurse Consultants (CNCs) to help guide ACP conversations.  

“CNCs can assist consumers and their families to understand the types of health care decisions that may be needed, support the completion of ACP documents and assist with sharing completed documents with the Statewide Office of ACP (Queensland Health), GPs and other health professionals involved in care,” Christine says. 

The clinics will be held at locations throughout the Metro South Health region including Woolloongabba, Beenleigh, Browns Plains, Eight Mile Plains, Inala and Wynnum-Manly. Details are included below or you can view the flyer.
